Role Summary
We are seeking a licensed Concierge Physician to provide high‑quality, patient‑centered medical care and clinical oversight to our clients in-home and via telehealth. The physician will work collaboratively with care managers, nurses, allied practitioners, and our vetted small‑business partners to ensure safe transitions, individualized care plans, and integrated wellness services.

Contract & Compensation
Independent contractor agreement. Scheduling and compensation negotiated based on hours, scope, and clinical responsibilities. Competitive hourly or per-visit rates, with premium pay for after‑hours/on‑call work.
Reimbursement for travel within Marin (mileage) and administrative support provided.

Requirements

  • Key Responsibilities

    • Provide in‑home and telehealth primary care, urgent visits, and episodic assessments for older adults and medically complex clients.

    • Conduct comprehensive evaluations, medication reviews, and advance care planning discussions.

    • Collaborate with hospital discharge teams for rapid-response transitions and post-discharge follow-up.

    • Support care planning for clients with dementia and complex chronic conditions, offering practical recommendations and caregiver coaching.

    • Provide clinical oversight and guidance to care managers, home health teams, and contracted caregivers.

    • Document clinical encounters in client records and communicate care plans to the interdisciplinary team and family members.

    • Participate in case conferences, training sessions, and quality improvement activities as available.

    • Maintain professional standards, licensure, malpractice insurance, and compliance with HIPAA.


    Qualifications

    • MD or DO with active California medical license in good standing.

    • Current DEA registration (as applicable).

    • Minimum 3 years clinical experience caring for older adults, geriatrics, palliative care, or primary care preferred.

    • Experience with home visits, transitions of care, or concierge/house-call practice strongly preferred.

    • Excellent communication skills and ability to collaborate with interdisciplinary teams.

    • Comfortable with telehealth platforms and electronic medical records.

    • Malpractice insurance and willingness to provide certificate of insurance.
